To fill out an adoption application, select one of the following:

Download our Adoption Application in Word Document Format
by right clicking on the link above and selecting
"Save Link To Disk" or "Save Target As" or "Save Link As"

Save it with the .doc extension or the .rtf extension
otherwise we Cannot Process your Application.

Once you have completed it, save it and and email it
as an attachment to: Small Dog Rescue
